All 50 U.S. states (State Health Departments), the District of Columbia, 2 American Indian Tribes (Santee Sioux; Kickapoo Tribe, KS), 5 U.S. Territories (American Samoa, Guam, Commonwealth of Northern Mariana Islands, Puerto Rico, U.S. Virgin Islands) and 3 Freely Associated States (Federated States of Micronesia, Republic of Marshall Islands, and Republic of Palau) are eligible to apply.
